Professor Underwood trained in Manchester. He has worked as a GP in Lusaka, Manchester, Tower Hamlets, and latterly in Coventry. His main area of research is into the improved diagnosis and management of musculoskeletal disorders, particularly back pain and osteoarthritis. He is now based in Warwick Clinical Trials Unit, of which he was previously the director. His recent and current work includes randomised controlled trials of robotic assisted hip and knee replacement, meniscal allograft transplantation, rehabilitation for long-covid, supported opioid tapering, and supportive self-management for people with chronic headache disorders. He has previously led a pilot study of facet joint injections that failed to recruit adequately. He is co-author on multiple systematic reviews of intervention for pain. In 2023 he was co-chair of WHO back pain guideline development group.
